This presentation will help attendees understand the role that a healthy diet plays in every facet of their life and provide tips for building a healthier diet and maintaining a healthy lifestyle.

Teladoc Diabetes Management*
As part of your Teladoc benefit, you can get an advanced blood glucose meter and as many strips and lancets as you need – all at no cost to you. Get access to the tools you need for diabetes management, including:
- Personalized tips with each blood sugar check
- One-on-one health coaching
- Real-time support when you’re out of range
- Strip re-ordering right from your meter
Teladoc services are available 24/7 via video, phone, or app. Teladoc appointments may be as quick as 10 to 15 minutes. Doctors can even send a prescription to your pharmacy if needed.
Quantum Health*
A few small steps today can make a big difference in the long run. Don’t forget to complete the following items by December 31 in order to earn your wellness incentive:
- Complete the wellness assessment
- Get your annual physical or preventive screening.
At your physical or screening, make sure to let your provider know that this is your preventive exam so they code the visit properly. You will get credit for this activity once the claim is processed. Please note that retroactive credits or late submissions will not be accepted.
