Pets are family, too. With pet health insurance coverage from Pets Best, a leading US provider, pet parents don’t have to decide between their pet’s care and over-extending personal finances.
Pets Best coverage helps you afford the best course of treatment, protects against major financial setbacks, and can give you peace of mind so you can focus on caring for your pet without worrying about the financial burden. You’ll get reimbursed for veterinary bills when your dog or cat is sick or injured.
Coverage Options
You have three coverage options:
- Silver: Covers accidents, illnesses, cancer, hereditary conditions, emergency care, surgeries, and prescription medications
- Gold: Covers everything Silver coverage does, plus accident and illness exam fees, rehabilitative care, acupuncture, and chiropractic care
- Platinum: Covers everything Gold coverage does, plus routine care.

Critical Illness Insurance
Critical Illness Insurance helps with out-of-pocket expenses for covered illnesses like a heart attack or cancer.
If you experience a covered critical illness, the plan pays a lump-sum benefit on initial diagnosis. You select either $15,000 or $30,000 as your benefit amount and can use the money any way you choose. This benefit is in addition to other insurance coverage you may have.
- Critical Illness benefits are paid directly to you (unless otherwise assigned) as a lump sum, based on diagnosis or event.
- Cover yourself, your spouse/domestic partner, and your children without answering health questions. Your covered spouse/domestic partner can have coverage of 50% of your benefit and children are automatically covered at 50% of your benefit at no additional cost.
- Additional and reoccurrence benefits are also included.
- You do not have to be terminally ill to receive the benefit.
- Includes a $50 annual Health Screening benefit that provides for certain diagnostic tests (for example, Pap smear, colonoscopy, or mammography).

Hospital Indemnity Coverage
Hospital Indemnity coverage provides benefits in addition to your medical plan if you or a covered family member is hospitalized. This may help you avoid dipping into savings or borrowing to pay out-of-pocket expenses, like transportation, meals, help with child care, or time away from work.
Hospital Indemnity benefits include:

- Hospital Intensive Care Benefit
- Intermediate Intensive Care Step-Down Unit

Accident Insurance
Accident Insurance is a great way to supplement your medical insurance. Not meant to be a stand-alone medical plan, this coverage provides extra dollars to help pay out-of-pocket expenses for covered injuries and accidents — occurring on or off the job.
Coverage is available for individuals and families. No health questions or physical exams are required. The plan pays specified dollar amounts to help pay for a wide variety of injury and accidents expenses, including:
- Hospitalization
- Doctor’s office visits
- Physical therapy
- Emergency room
- Lodging for family
There’s even a $50 annual benefit for getting a preventive screening.

401(k) Income Savings Plan

Save for your retirement with the Hertz 401(k) plan. Hertz offers the Income Savings Plan (the “Savings Plan” or the “401(k)”) to help you save for retirement and plan for your future. For more detailed information about the 401(k), read the Income Savings Plan Summary Plan Description (SPD).
- You can participate in the 401(k) beginning the first day of the calendar month following 30 days of employment.
- All enrolled participants can get personalized planning and advice from Fidelity.
- The Safe Harbor Notice describes the Safe Harbor Matching Contribution and how participants become eligible to receive this contribution.

Contribution Limits
The 2026 IRS contribution limit is $24,500 ($23,500 in 2025). If you are age 50 or older by the end of the calendar year, you can contribute an additional $8,000 ($7,500 in 2025) and if you are between age 60 and 63 by the end of 2026, the catch up contribution increases to $11,250.
Hertz Matching Contribution
Hertz will match your contributions dollar for dollar on the first 3% you contribute and 50 cents per dollar on the next 2% you contribute. This means your account is credited with a 4% matching contribution if you contribute 5% or more. As a safe harbor 401(k) plan, your employee and employer contributions are immediately 100% vested.

Long-Term Disability (LTD) Insurance
Paycheck Insurance in the Event You Can't WorkLong-Term Disability (LTD) Coverage
If you suffer a disabling injury or illness, you may be unable to work for an extended period of time. Hertz offers optional income protection coverage under the Long-Term Disability (LTD) plan. LTD replaces a portion of your basic monthly earnings if you continue to be disabled and unable to work after exhausting Short-Term Disability (STD) benefits.
How It Works
- You may pay for LTD coverage with after-tax or pre-tax contributions. When you elect after-tax contributions, any LTD benefit is not taxed. If you choose LTD coverage on a pre-tax basis, any benefit is subject to income tax.
- You receive LTD benefit payments after you have been disabled for 180 days on an approved period of STD. The benefit is a percentage of your base pay at the time you became disabled, depending on the option you elect and subject to benefit minimums and maximums. Your LTD benefit is reduced by benefits you receive or are eligible to receive from other sources, such as Social Security and workers’ compensation.
- Benefits continue for the maximum benefit period, as long as you remain disabled.

Evidence of Insurability
Evidence of Insurability (EOI) is how you provide proof of good health to The Hartford.
- If you’re a new hire and enroll when first eligible, you can elect either coverage option without EOI.
- If you’re a current employee and enroll after you are first eligible or change your coverage, EOI is required.
Filing a Claim for LTD Benefits
You may file a claim for LTD benefits by telephone or online after you know you’ll be absent from work for seven (7) consecutive days or more due to an illness or injury.
- You must provide proof of your disability when you file.
- If your claim is approved, you’ll receive disability pay for the period of your disability minus the waiting period.
- If your claim is denied, you have the right to appeal.

Short-Term Disability (STD) Insurance
Income Protection for Unforeseen Injuries or IllnessesShort-Term Disability (STD) Coverage
Short-Term Disability (STD) is paycheck insurance that protects a portion of your income in the event of a short-term illness, pregnancy, or injury. STD benefits are payable for up to 26 weeks (after a 7-day elimination period) for an approved disability leave.

Non-Exempt (Hourly) Employees
Non-exempt (hourly) employees automatically receive a core STD benefit of 50% of your weekly base pay up a $150 weekly maximum. You can also purchase supplemental coverage of:
- 50% of your weekly base pay up to a $500 weekly maximum, or
- 60% of your weekly base pay up to a $1,000 weekly maximum.
Important information if you live in CA, NY, HI, RI, NJ or any other state with a state disability program:
- California employees receive STD benefits through the state disability program instead of Hertz.
- New York employees may purchase STD coverage to supplement the $170 weekly maximum benefit provided by the state disability program.
- Hawaii, Rhode Island, and New Jersey employees may purchase the 60% option to supplement your state disability program.
- If you live in a state with a state disability program, you are responsible for initiating your claim with the state program in addition to or instead of Hertz disability coverage.
Exempt (Salaried) Employees
Exempt (salaried) employees are eligible to receive a salary continuation benefit for approved disability leaves. Salary continuation benefits consist of full base pay for a specified period of time, and then partial (half) base pay for a specified period of time, depending upon your length of service with the company. The policy is available at hertz.policytech.com.
Salary continuation benefits are integrated with state disability insurance benefits.
Pre-Existing Conditions
A “pre-existing condition” is a sickness or accidental injury in the three months before your coverage takes effect for which you:
- received treatment, consultation, care, or services, or
- took prescription medication or had medications prescribed.
The Hartford will not pay benefits for a disability resulting from a pre-existing condition until you have been actively at work for 12 consecutive months after your coverage takes effect.

Life Insurance
Hertz provides Life Insurance coverage for you and your dependents, administered by The Hartford.
- Hertz-paid Basic Life Insurance provides coverage equal to one-times your base annual pay, at no cost to you.
- You may elect to purchase voluntary Supplemental Term Life Insurance coverage of up to six-times your annual base salary, up to a total of $3 million for both Basic and Supplemental coverage.

Evidence of Insurability (EOI) Requirement
Evidence of Insurability (EOI) is how you provide prÂÂÂoof of good health to The Hartford.
- If you’re a new hire and enroll when first eligible, you can elect coverage up to the lesser of five times your annual base pay or $300,000 without EOI.
- If you’re a current employee and enroll after you are first eligible or increase your Supplemental coverage by more than one times annual base pay, EOI is required for coverage above five times annual base pay.

AD&D Insurance
Accidental Death & Dismemberment (AD&D) Insurance coverage provides financial protection for you and your family in the event of a covered accidental injury or death.
- You may enroll in AD&D coverage when you’re first hired or during annual open enrollment. Evidence of Insurability (EOI) is not required.
- You may purchase coverage for yourself up to 10 times your annual base pay, with a $750,000 maximum. If you enroll, you may also cover your spouse/domestic partner and any eligible dependent children.
- You pay for AD&D coverage with pre-tax dollars and you’ll need to designate a beneficiary. Use the Enroll/Log In button at the top of the page to add or change your beneficiary.
