Wellbeing Programs Included in Your Hertz Medical PlanThese programs are part of the Hertz medical plans. If you aren’t enrolled in medical coverage, you still have access to other Wellbeing Programs, including the Employee Assistance Program (EAP), Employee Resource Groups (ERGs), and online wellness classes from Perks at Work.
Through our mutual commitment to wellness, not only can we enhance our lives, but we’re better able to manage health care costs. The healthier you are, the less you and Hertz will spend on medical expenses throughout the year.
Care Coordinators
Quantum Health
When dealing with healthcare and benefits, it can be hard to know where to start. Quantum Health Care Coordinators will be your guides to navigate the healthcare system.
Care Coordinators can help you:
- Get answers to claims, billing and benefits questions
- Find in-network providers
- Verify coverage and get prior approval if needed
- Contact providers to coordinate your treatment
- Review your care options
- Replace ID cards
- Enroll in the tobacco cessation program
Wellness Reward
Hertz believes in the importance of regular health screenings and rewards employees who take steps to maintain good health. Employees who receive an annual preventive health screening and complete Quantum’s annual health risk assessment will be eligible to receive a wellness reward.
Annual Physical Exam Requirement
If you are enrolled in a Hertz medical plan, you – and if applicable, your spouse or domestic partner – must complete an annual, comprehensive adult physical exam to receive the $50 per month premium reduction reward for the next calendar year.
If you cover a spouse or domestic partner, they will also be required to complete these activities in order for you both to earn your rewards. Visit HealthyHertz.com to take your health assessment and track your progress.

Healthworks
You and your enrolled spouse/domestic partner can earn up to $350 by completing these activities:*
- Complete the online total health assessment to receive a $200 Visa reward card.
- Complete up to three healthy lifestyle programs to receive an additional $50 Visa gift card for each completed program. The healthy lifestyle programs are free, interactive, and personalized to empower you to take charge of your own health and reshape your lifestyle.

*Your health plan is committed to helping you achieve your best health. Rewards for participating in the Hertz Wellness Reward are available to all employees. If you think you might be unable to meet a standard for a reward under this program, you might qualify for an opportunity to earn the same reward by different means. Contact Human Resources and we will work with you (and, if you wish, with your doctor) to find a wellness program with the same reward that is right for you in light of your health status.
Teladoc Health
Virtual Health Care & Diabetes Management
If you have a minor illness, going to the emergency room can be expensive, urgent care may be unnecessary, and you may not be able to see your primary care physician right away. Teladoc saves you time and money by allowing you and your enrolled dependents to speak with a doctor anytime, anywhere.
Teladoc appointments can be accessed from your internet-connected device or by phone and may be as quick as 10 to 15 minutes. Doctors can even send a prescription to your pharmacy if needed.

Best Doctors® Network

Behavioral Health
Through Teladoc, you can also access behavioral health practitioners, including psychiatrists (MD/DO), psychologists (PhD), counselors, clinical social workers and therapists with masters degrees, and substance abuse counselors. The program offers diagnostic services, talk therapy, and prescription medication management, when appropriate.
You must complete a comprehensive medical history and assessment before receiving a consultation. The program treats issues such as stress, anxiety, depression, addiction, domestic abuse and grief counseling.

Omada
The Diabetes Management program will no longer be managed through Omada beginning Jan. 1, 2025. As part of our goal to help streamline and enhance our benefits offering, we are transitioning our diabetes management program to Teladoc. Through Teladoc, eligible members will continue to have access to diabetes management services along with personalized tip and expert coaching. Refer to the section above for more details.
The Hertz Omada program can help you and your adult family members manage type 2 diabetes. This personalized program offers the human support and digital tools you need to build healthy habits and feel in control of type 2 diabetes, including:
- A certified diabetes educator for tailored support and guidance
- Glucose meter, strips and scale to monitor progress (and keep for good)
- Online community for diabetes-specific encouragement and motivation
- Health tracking and lessons to demystify (and better manage) diabetes
- Endless flexibility with the mobile app and 24/7 online access

Tobacco Cessation
The Tobacco Cessation program is managed through Quantum Health as of Jan. 1, 2025.
Ready to quit or reduce your use of tobacco? Take advantage of the Tobacco Cessation program included in your Hertz medical plan. Over 50% of tobacco users who go through the program are tobacco-free after just six (6) months.
How it Works
If you or your spouse/domestic partner are ready to quit tobacco, the free Tobacco Cessation program can help.
Call 877-674-3045 to confidentially speak to a Quantum health coach. Your coach will work with you to create a quit plan and provide the support you need to succeed.
Eligibility
All employees and spouses/domestic partners enrolled in a Hertz medical plan are eligible for the Tobacco Cessation Program.
Confidentiality
All information that you share or discuss with a tobacco cessation coach is completely confidential. The Tobacco Cessation Program is administered by Quantum Health and your personal results will never be reported to Hertz.
Surgery Support Services
Carrum Health
When you need non-emergency surgery, understanding your options, finding a quality provider, setting up your appointment and coordinating post-care rehab can be a difficult process.
Hertz is pleased to offer surgery support services through Carrum Health. Carrum Health works with highly-rated centers of excellence (COE) around the country to offer members best-in-class surgery support from the time you need to schedule your surgery to your rehab. Hertz is offering this service to all enrolled medical members.

Utilizing Carrum is required if you are seeking bariatric surgery services. Using Carrum for all other eligible surgeries is voluntary.
If you do coordinate your surgery through Carrum, the out-of-pocket cost of your surgery will either be waived, if enrolled in the PPO or Copay Plan, or significantly reduced, if enrolled in the CDHP.

Hertz is pleased to partner with WINFertility to help support both fertility and maternity services. Regardless of your needs, Hertz wants to support you and your family.
Throughout your journey, your nurse advocate will offer specialized care and assist in answering any questions you may have around various services and treatments.

WINFertility
If you’re experiencing infertility, please know you are not alone. Hertz wants to support you and your family during this time. Your Hertz medical plan comes with free access to WINFertility. This program connects you with a fertility nurse advocate who can provide support and encouragement through your experience and help answer your questions.
WIN’s nurse advocates have expertise in understanding complex information such as common causes, testing and treatment options, as well as medication guidance. WIN’s nurse advocates can also help you navigate your benefits and select an in-network provider based on your unique goals, needs and preferences.
WINMaternity
Hertz’s partnership with WIN also includes broad maternity support. The goal of this program is to foster healthy pregnancies and births. Your nurse advocate will offer specialized care and assist with services such as prenatal health and nutrition, behavioral health, locating a specialist
if needed, postnatal care and access to doula services.
Bariatric Resource Services (BRS)
Optum
This program is going away after Dec. 31 and will transition to Carrum Health. Refer to the Carrum section above for more details.
Optum Bariatric Resource Services (BRS) provides you with access to a team of clinical experts who specialize in weight loss and bariatric surgery. You’ll receive complete support before, during and after bariatric surgery.
The BRS program is already a part of your benefits, so there’s no extra cost. If you are considering surgery, please call to learn more, connect with a nurse, and locate a Center of Excellence provider near you.
Bariatric Resource Services nurses can help you:
- Find high quality Centers of Excellence network providers to make sure your surgery goes as smoothly as possible.
- Learn about surgical options for best possible outcomes.
- Find resources to help you meet your pre-surgical requirements.
- Understand how nutrition and fitness can help you achieve success.
- Adjust to and maintain lifestyle changes and avoid complication after surgery.

SWORD Health’s virtual physical therapy program specializes in prevention and management of chronic back, joint, and muscle pain. Hertz medical plan members are eligible to participate at no cost. As part of the program, you will be assigned a designated physical therapist that will tailor a program specific to your needs, allowing for high-quality, engaging, and personalized treatment that aims to help avoid surgery and drastically reduce healthcare costs.
