Plan A is a Preferred Provider Organization (PPO) that offers in-network coverage only. You pay copays or coinsurance, depending on the service. If you see an out-of-network provider, you will not have coverage. You can use the Health Care Flexible Spending Account (HCFSA)

to pay for covered services with pre-tax dollars.
